1. Approved Electrical Plan Indicating 1) Transformer Size or Total Connected Load, whichever is applicable, 2) Voltage Drop 3) Fault Calculation - (One Copy A3, Signed and Sealed by PEE)

2. Site Development Plan Indicating the Location of Service Entrance

3. PEZA-OBO Electrical Permit

4. PEZA-OBO Certificate of Final Electrical Inspection

PROOF OF BUSINESS REQUIREMENTS

1. SEC or DTI Registration

2. Articles of Incorporation

3. BIR 2303

4. PEZA Certificate of Registration

5. PEZA Zero-Rated Certificate (Latest)

6. Lease/Sub-lease Agreement with Building Owner/Developer

COMPANY REPRESENTATIVE

​

1. Photocopy of 1 Valid Government ID

2. Board Resolution or Secretary’s Certificate (Appointing Representative of the Company to Sign Documents/Contracts)

PAYMENT(S)

1. Bill Deposit (This shall be computed based on the applied Load as indicated in the Approved Electrical Plan and will only be collected upon Completion of All Above Requirements)

2. Arrears of the Previous Owner/Locator of the Premise (As Applicable)
